
A state-led review reveals that nearly 40 hard-copy files related to foster care, adoption and child-care services are missing from Richmond's Department of Social Services.
Stephen Harms, the interim deputy chief administrative officer for Human Services, says the department's poor record-keeping practices is a weakness and a flaw that must be corrected immediately.
Harms' predecessor, Carolyn Graham, resigned in December amid an inquiry into child-welfare practices in the city. Graham will get almost $60,000 in severance pay and compensation after stepping down.
